Our Dream Makeover Giveaway is not limited to current Studio 921 clients. Anyone is eligible to win.
ELIGIBILITY:
Search is open to nominators and nominees who are each residents of the state of Maryland. Nominees must be 21 or older. The immediate family and household members of Studio 921 employees are not eligible to nominate or be nominated. "Immediate family members" shall mean parents, step -parents, children, step-children, siblings, step-siblings, or spouse. "Household members" shall mean people who share the same residence at least three months a year.
TO BE CONSIDERED FOR PARTICIPATION:
Send us a short letter, one page or less describing your friend or loved one and why you think they should be the recipient of Studio 921’s dream makeover. Include names, addresses, phone numbers, and recent photos of your nominee. All photos must have been taken within the last 12 months. Submissions must be the sole property and original work of nominator; nominator must have all rights to reproduce and distribute the photo submitted. Any submission not meeting these requirements will be immediately disqualified. Any submissions containing materials which may be deemed indecent, inappropriate, objectionable or otherwise unfit for use in publication will not be considered.

Studio 921 can provide no technical support and accepts no responsibility for insuring the receipt of your e-mail and photo. If you experience any difficulties trying to submit your materials via e-mail, we recommend you send your letter and photo via U.S. mail to us at the above address.
All submissions (whether mailed or submitted electronically) must be received in our salon by 12 pm on May 31, 2008 to be eligible. Letters and photos will not be acknowledged or returned. All submissions (including photos) become the property of Studio 921 and may be used without compensation in any manner, and, in any media, for publicity, promotional and/or other purposes, to the extent permitted by law.
SELECTION:
Submission which might be featured in area publications may be selected based on: the compelling nature of the letter and/or the specialness of the nominee. Studio 921’s staff may conduct telephone interviews with nominators and nominees. Submission which might be featured will be selected by representatives of the Studio 921 staff. All decisions of the Studio 921 staff are final and binding.
NOTIFICATION OF POTENTIAL GUESTS:
Nominee may request specific makeover essentials. Requests are subject to adjustments and restrictions imposed by Studio 921, as appropriate, for reasons of cost, practicality or otherwise. Retail value of those items necessary to make the nominee's Dream Makeover come true will vary depending on the person. The nominators of any selected nominees and the nominees themselves will be notified by telephone during the week of June 5th 2008. Each Nominator must sign a Release and Indemnification form within the time period specified by Studio 921. Failure to timely comply with any requirement of these terms and conditions will result in the submission being deemed ineligible for consideration and any further opportunity to participate in this search.
Claiming the Dream Makeover Giveaway
Dream Makeover services must be scheduled within 6 months of notification to the nominee. Services must be redeemed on the day designated by Studio 921 and in the order chosen by Studio 921.
RELEASES AND MISCELLANEOUS TERMS:
Nominators and Nominee's agree to release, discharge and hold harmless Studio 921 Salon and Medi Day Spa, advertising and promotional companies, any other vendors participating in this Dream Makeover giveaway and their respective owners, employees, agents and representatives ("Released Parties") from and against any liability, damage or expense arising from or relating to participation in this search.
Submitting materials for possible use constitutes permission to use nominator's and nominee's name, likeness, photograph, biographical information and any materials submitted as part of this search for programming, publicity, promotional and/or other purposes in any manner, and in any media, without compensation, in perpetuity to the extent permitted by law.
Studio 921 is not responsible for late, lost, illegible, postage due, mutilated, incomplete, delayed or misdirected mail; for theft or destruction or unauthorized access to or alterations of submitted materials; for technical, network, telephone equipment, electronic, computer, hardware or software delays, interruptions or malfunctions of any kind, inaccurate transmission of or failure to receive submitted information by Studio 921, or human error; for any injury or damage to submitters or any other person's computer related to or resulting from entering or downloading any materials in connection with the search.
CAUTION:
Any attempt to deliberately damage any Web site or undermine the legitimate operations of this search may be a violation of the criminal and civil laws, and should such an attempt be made, Studio 921 or its designated representative, reserves the right to seek damages from any person associated with such actions to the fullest extent permitted by law. Studio 921 or its designated representative, reserves the right to cancel or modify the search if it should become compromised in any way or if it becomes technically corrupted.
